Your Initial Visit

Our Aim Is For Your Initial Visit To Be One Of Comfort And Acknowledgment Of Your Personal Dental Needs.
By completing the patient forms online prior to your first dental appointment, we can make your visits more efficient while maintaining the excellent level of care you expect.
At the time of your initial consultation, kindly provide the following information to help us:
Your referral form and, if necessary, any X-rays.
A list of medications you are currently taking.
If you have medical or dental insurance, please bring the required paperwork/insurance card with you. This will save time and allow us to assist you with claims processing.

18+All patients under the age of 18 must be accompanied by a parent or guardian for the consultation appointment.

If you have a medical condition that needs to be addressed (e.g., diabetes, high blood pressure, artificial heart valves, and joints, etc.) or if you are on any medication (e.g., heart medications, aspirin, etc.) or require medication prior to dental cleanings, please alert our team.
